To propose a change to the WFOP, submit a completed WFOP Change Form template
Change Form to the WFOP Coordinator. Please make edits to existing text in "Track Changes" to ensure your proposed changes are captured accurately (utilize 2017 files).

Change forms approved after March 1, 2018, will not be included in the printed version of the 2018 WFOP.
